- This E-Coupon cannot be exchanged for cash.
- This E-Coupon is valid only for the specified check-up program and cannot be altered.
- Valid only for outpatient (OPD) clients and individuals who are self-reliant.
- The check-up package does not include doctor’s fees if a specialist is specifically requested, or fees for radiological exams.
- The check-up package does not include additional treatment or medication fees if prescribed by the doctor.
- Please make an appointment in advance and fast for 8–10 hours prior to the check-up (sipping water is allowed).
