1. Services are reserved for customers who present the E-Coupon only.
2. The hospital reserves the right to present laboratory test details and pricing.
3. If the coupon is not presented, the customer will be required to pay the full price of the health check-up package.
4. This coupon cannot be refunded or exchanged for cash.
5. This coupon is valid only for the services listed and cannot be changed or modified.
6. This coupon is applicable for outpatient (OPD) services and self-ambulatory patients only.
7. The health check-up package does not include doctor’s fees in cases where a specific specialist is requested for consultation or radiology examination.
8. The health check-up package does not include additional treatment or medication costs if prescribed by a physician.
9. Please make an appointment in advance and fast for 10–12 hours before the examination (drinking plain water is allowed).

11. Services are available at the Health Design Center (HDC), 5th Floor, Building R,Bangkok Hospital (Soi Soonvijai), everyday from 07:00 – 14:00.
Note:
1. In cases that customers request to cancel an order for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund or cancel the product at its discretion.
2. The receipt amount displayed is the actual payment received, excluding any promotional discount.
3. After completing the purchase, customers will receive an online receipt only. If a physical receipt is required on the day of the visit, it can be requested from the cashier.
