2.E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้
3.E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้นไม่สามารถเปลี่ยนแปลงได้
4.สำหรับใช้บริการผู้ป่วยนอกเท่านั้น / อัตราดังกล่าวรวมค่าแพทย์ผู้ป่วยนอกแล้ว
5.ชุดตรวจสุขภาพ ไม่รวมค่าแพทย์ในกรณีเจาะจงแพทย์ผู้เชี่ยวชาญในการตรวจสุขภาพและตรวจทางรังสี
6.ชุดตรวจสุขภาพ ไม่รวมค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

วันหมดอายุ E-Coupon
1. E-Coupon จะมีอายุการใช้ถึงวันที่ 30 พฤศจิกายน พ.ศ. 2567
2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้
3. แพ็กเกจราคานี้ สามารถสั่งซื้อได้ตั้งแต่ วันที่ 27 กรกฎาคม - 18 สิงหาคม พ.ศ. 2567
หมายเหตุ
1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Health Plaza Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ
2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

2.This E-Coupon cannot be exchanged for cash.
3.E-Coupon is only applied to specific items as mentioned and changes cannot be made.
4.These packages are for outpatients who are physically dependent.
5.These packages exclude doctor fees in case additional specialists are required.
6.These packages exclude treatment fees or medications prescribed by the doctors, if any.
7.For further information or to schedule an appointment, please contact Vaccine Clinic, 1st floor, D Building,
Bangkok Hospital, every day from 08:00 AM to 04:00 PM or Contact Center at

8.Services are only available at Vaccine Clinic, 1st floor, D Building, Bangkok Hospital.
9.This item is for service recipients aged 15 years and above.
10.This price is for Thai citizens and expatriates only.
Expiration Date of E-Coupon:
1. The E-Coupon is valid until November 30, 2024.
2. If the E-Coupon expires, the hospital reserves the right to deny service access, and the E-Coupon cannot be extended or refunded.
3.This package price can be ordered from July 27, 2024, to August 18, 2024.
Note:
1. In case customers request cancellation of the order after the Health Plaza Guarantee period without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.
2. The receipt amount displayed is the actual payment received, excluding any promotional discounts.
