รายการตรวจประกอบด้วย
1. วัดการมองเห็น + ตรวจสายตาสั้น ยาว เอียง ด้วยคอมพิวเตอร์ (VA + Auto – refraction)
2.วัดความดันตาด้วยเครื่อง (Tonometer)
3.หยอดยาขยายม่านตา (Pupil Dilatation)
4.ถ่ายภาพขั้วประสาทตา + จอประสาทตา (Fundus Imaging)
5.สแกนภาพขั้วประสาทตา + จอประสาทตา (Optical Coherence Imaging)
6. พบแพทย์เพื่อตรวจสุขภาพตาและรับคำปรึกษา
*กรณีที่ผู้ป่วยตรวจครั้งแรก จะได้พบจักษุแพทย์เพื่อตรวจตาก่อนขยายม่านตา และส่งตรวจรายการที่ 1-5 เป็นการตรวจโดยเทคนิเชี่ยน และพยาบาลหยอดขยายม่านตา และกลับเข้าไปพบแพทย์อีกครั้ง
** หากมีการตรวจวิเคราะห์เฉพาะโรคเพิ่มเติม จักษุแพทย์จะเป็นผู้ให้คำแนะนำ และแจ้งค่าใช้จ่ายให้ทราบก่อนตรวจ **
คำแนะนำเพิ่มเติม
- ระยะเวลาในการตรวจทั้งหมดประมาณ 1 – 1.5 ชั่วโมง
- ไม่ต้องงดน้ำงดอาหารก่อนตรวจ
- เนื่องจากในรายการตรวจนี้ มีการหยอดยาขยายม่านตา หลังตรวจผู้ป่วยจะมีอาการตาพร่ามัว สู้แสงไม่ได้ เนื่องด้วยฤทธิ์ของยาขยายม่านตา ซึ่งจะหมดฤทธิ์ภายใน 4-6 ชั่วโมงห้ามผู้ป่วยขับรถกลับเอง แนะนำให้มีญาติคอยดูแลในการเดินทางกลับบ้าน
เงื่อนไขการรับบริการ
1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำ E-Coupon มาแสดงเท่านั้น
2. ในกรณีที่ไม่นำ E-Coupon มาแสดง ท่านต้อง ชำระค่ามัดจำ ตามราคาชุดตรวจ
3. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้
4. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้
5. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น
6. ชุดตรวจสุขภาพ ไม่รวม ค่าแพทย์ในกรณีเจาะจงแพทย์ผู้เชี่ยวชาญในการตรวจสุขภาพและตรวจทางรังสี
7. ชุดตรวจสุขภาพ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

หมายเหตุ
1. ในกรณีที่ลูกค้าประสงค์จะยกเลิกคำสั่งซื้อด้วยเหตุผลที่ไม่เกี่ยวข้องกับโรงพยาบาลหรือเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า
2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ

Note:
1. In cases that customers request to cancel an order for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund or cancel the product at its discretion.
2. The receipt amount displayed is the actual payment received, excluding any promotional discount.
3. After completing the purchase, customers will receive an online receipt only. If a physical receipt is required on the day of the visit, it can be requested from the cashier.
