4.สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

5.แพ็คเกจนี้ไม่รวมค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

7. สามารถรับบริการได้ที่คลินิก​ทันตกรรม​กรุงเทพ ชั้น 2 หรือ ชั้น 3 อาคารทันตกรรมรพ.กรุงเทพ (หน้าปากซอยเพชรบุรี 47)

เปิดบริการ ทุกวัน 09.00 - 20.00น.

8.ทำนัดได้เฉพาะทันตแพทย์ที่ดูแลรอยโรคในช่องปากเท่านั้น

 

หมายเหตุ

1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Shopee Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ

2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

2.This E-Coupon cannot be exchanged for cash.

3.This E-Coupon is only applied to specific items mentioned on the coupon, and changes  cannot be made.

4.The services is only for Outpatients with physical independence.

5.These packages do not include the fees of other additional treatment or medications  prescribed by the doctors,if needed any.

Note:

1. In case customers request cancellation of the order after the Shopee Guarantee period without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.

2. The receipt amount displayed is the actual payment received, excluding any promotional discounts.

1.    หลังจากสั่งซื้อ คุณจะได้รับ sms ที่มีคูปองอิเล็กทรอนิกส์
2.    โทรนัดหมายล่วงหน้าอย่างน้อย 3 วัน ก่อนเข้ารับบริการ โทร Contact Center โทร.1719 หรือ Add Line @bangkokhospital
3.    ก่อนวันเข้ารับบริการ กรุณาเข้าไปที่ คำสั่งซื้อของฉัน -> ที่ต้องได้รับ และคลิกปุ่ม ตรวจสอบและยอมรับ
4.    แสดงคูปองอิเล็กทรอนิกส์ ให้กับเจ้าหน้าที่ในวันที่รับบริการ

1. After successful payment, you will receive an SMS with an e-coupon.
2. Please schedule an appointment at least 3 days in advance before receiving the service by calling the Contact Center at 1719 or add line @bangkokhospital
3. Before the service date, please go to "My Orders" -> "To Receive" and click the "Check and Accept" button.
4. Present the e-coupon to the staff on the day of the service.
