2. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้

3. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

4. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

5. E-Coupon นี้ใช้เฉพาะการตรวจหาคราบจุลินทรีย์ เอกเรย์ด้านประชิดพร้อมขจัดคราบจุลินทรีย์ และขูดหินปูนด้วยเครื่อง GBT เท่านั้น ไม่รวม การรักษาโรคเหงือกซึ่งทันตแพทย์จะเป็นผู้พิจารณา

6. ชุดตรวจสุขภาพไม่รวมค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

เปิดบริการ ทุกวัน 09.00 - 20.00 น.

หมายเหตุ
1. ในกรณีที่ลูกค้าประสงค์จะยกเลิกคำสั่งซื้อด้วยเหตุผลที่ไม่เกี่ยวข้องกับโรงพยาบาลหรือเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า
2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ

2.This E-Coupon cannot be exchanged for cash.

3.This E-Coupon is only applied to specific items mentioned on the coupon, and changes cannot be made.

4.The services is only for Outpatients with physical independence.

5. This E-Coupon is only applicable for bitewings x-rays, teeth scaling and polishing, and Guided

Biofilm Therapy. It does not include periodontal treatment (treatment for gum disease). The dentist will assess and determine an appropriate treatment for gum disease separately.

6.These packages do not include the fees of other additional treatment or medications  prescribed by the doctors,

if needed any.

1. In cases that customers request to cancel an order for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund or cancel the product at its discretion.
2. The receipt amount displayed is the actual payment received, excluding any promotional discount.
3. After completing the purchase, customers will receive an online receipt only. If a physical receipt is required on the day of the visit, it can be requested from the cashier.
