เงื่อนไขการรับบริการ
1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำคูปองมาแสดงเท่านั้น
2. คูปองนี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้
3. คูปองนี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้
4. สำหรับใช้บริการผู้ป่วยนอกเท่านั้น / อัตราดังกล่าวรวมค่าแพทย์ผู้ป่วยนอกแล้ว
5. ชุดตรวจสุขภาพ ไม่รวม ค่าแพทย์ในกรณีเจาะจงแพทย์เฉพาะทางในการตรวจสุขภาพและตรวจทางรังสี

8. การใช้แพ็กเกจเป็นไปตามเงื่อนไขตามรายการที่กำหนดเท่านั้น สำหรับส่วนเกินเรื่องโรคประจำตัวของผู้ป่วย ภาวะแทรกซ้อน ค่าใช้จ่ายอื่นๆ นอกเหนือที่กำหนดไว้ รวมถึงการรักษาตัวเกินจากที่กำหนด ผู้ป่วยต้องชำระส่วนเกินตามจริงและหักส่วนลด (ถ้ามี)
9. จำนวนครั้งในการตรวจติดตามเป็นไปตามที่ระบุในแพ็กเกจ 12 ครั้ง
10. รวมการอบรมครรภ์คุณภาพไม่จำกัดจำนวนครั้ง

หมายเหตุ
1. ในกรณีที่ลูกค้าประสงค์จะยกเลิกคำสั่งซื้อด้วยเหตุผลที่ไม่เกี่ยวข้องกับโรงพยาบาลหรือเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า
2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ
3. หลังการสั่งซื้อ ลูกค้าจะได้รับใบเสร็จในรูปแบบออนไลน์เท่านั้น หากต้องการใบเสร็จตัวจริงในวันที่เข้ามารับบริการ สามารถแจ้งขอได้ที่เจ้าหน้าที่การเงิน

1. Service is reserved exclusively for customers who present the E-Coupon.
2. This E-Coupon cannot be exchanged for cash.
3. E-Coupon is only applied to specific items as mentioned and changes cannot be made.
4. These packages are for outpatients who are physically dependent.
5. These packages exclude doctor fees in case additional specialists are required.
6. These packages exclude treatment fees or medications prescribed by the doctors, if any.
7. Eligibility for the package is subject to the evaluation and discretion of the attending obstetrician-gynecologist, based on the medical condition of each individual.
8. The package is applicable only under the terms and scope specified. Any additional expenses resulting from pre-existing medical conditions, pregnancy-related or unrelated complications, or hospitalization beyond the coverage period shall be the responsibility of the patient, based on actual incurred costs and applicable discounts (if any).
9. The package includes 12 follow-up visits, as specified.
10. Unlimited participation in antenatal education classes is included.
11. Please make an appointment at least 3 days in advance.

13. Services are available at Women's Health Center, 2nd floor, D Building, Bangkok Hospital, every day from 08:00 AM to 05:00 PM
14. The package is available to Thai nationals, expatriates, and fly-in international patients.
Note:
1. In cases that customers request to cancel an order for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund or cancel the product at its discretion.
2. The receipt amount displayed is the actual payment received, excluding any promotional discount.
3. After completing the purchase, customers will receive an online receipt only. If a physical receipt is required on the day of the visit, it can be requested from the cashier.
