เงื่อนไขการรับบริการ

1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำคูปองมาแสดงเท่านั้น

2. คูปองนี้ไม่สามารถคืนหรือแลกเปลี่ยนเป็นเงินสดได้

3. คูปองนี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้ 

4. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

5. สามารถใช้บริการความงามได้ ภายในเวลา 1 ปี นับจากที่ได้ลงทะเบียนการใช้คูปองภายในเวลาที่กำหนด

6. ไม่รวม ค่ารักษา ค่ายา หรือหัตการอื่นๆ ที่นอกเหนือจาก Package 

7. ขอสงวนสิทธ์การโอนสิทธิ์ให้ผู้อื่นใช้คูปองแทน กรุณาแนบสำเนาบัตรประชาชนของผู้โอนและระบุลายเซ็นยินยอมมาแสดงในวันที่มาใช้บริการครั้งแรก

9. สามารถรับบริการได้ที่ ศูนย์ผิวหนังและความงามกรุงเทพ ชั้น 5 อาคาร Bangkok Plaza รพ.กรุงเทพ วันจันทร์ - ศุกร์ เวลา 08.00 - 20.00 น. วันเสาร์ - อาทิตย์ เวลา 08.00 - 17.00 น

วันหมดอายุ E-Coupon

1. E-Coupon มีอายุการใช้งานภายในวันที่ 31 ตุลาคม พ.ศ. 2568

2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้

3. แพ็กเกจราคานี้ สามารถสั่งซื้อได้ถึง 17 สิงหาคม พ.ศ. 2568  เท่านั้น

หมายเหตุ
1. ในกรณีที่ลูกค้าประสงค์จะยกเลิกคำสั่งซื้อด้วยเหตุผลที่ไม่เกี่ยวข้องกับโรงพยาบาลหรือเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า
2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ
3. หลังการสั่งซื้อ ลูกค้าจะได้รับใบเสร็จในรูปแบบออนไลน์เท่านั้น หากต้องการใบเสร็จตัวจริงในวันที่เข้ามารับบริการ สามารถแจ้งขอได้ที่เจ้าหน้าที่การเงิน

1. This service is reserved for those who present an E-Coupon only.

2. This E-Coupon cannot be exchanged for cash.

3. E-Coupon is only applied to specific items as mentioned and changes cannot be made.

4. These packages are for outpatients who are physically dependent. 

5. You are eligible to utilize beauty services within a one-year period starting from the date of coupon registration, as stipulated in the designated timeframe.

6. This package does not include treatment fees, medication costs, or any procedures beyond the package. 

7. We retain the right to transfer coupon privileges to another person. Please provide a copy of the transferee's ID and authorized signature for presentation on the day of their first service utilization.

9. Services are available at Skin and Aesthetics Center, 5th floor, Bangkok Plaza Building, Bangkok Hospital. Operating hours are Monday to Friday from 08:00 AM to 08:00 PM and Saturday to Sunday from 08:00 AM to 05:00 PM.

10. The price is applicable to both Thai nationals and expatriates.

Note:
1. In cases that customers request to cancel an order for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund or cancel the product at its discretion.
2. The receipt amount displayed is the actual payment received, excluding any promotional discount.
3. After completing the purchase, customers will receive an online receipt only. If a physical receipt is required on the day of the visit, it can be requested from the cashier.
