เงื่อนไขการรับบริการ

1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำ E-Coupon มาแสดงเท่านั้น

2. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้

3. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

4. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

5. E-Coupon นี้เฉพาะการขูดหินปูนเท่านั้น ไม่รวม การรักษาโรคเหงือก ซึ่งทันตแพทย์จะเป็นผู้พิจารณา

6. ชุดตรวจสุขภาพ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

เปิดบริการ ทุกวัน 09.00 - 20.00 น.

หมายเหตุ
1. ในกรณีที่ลูกค้าประสงค์จะยกเลิกคำสั่งซื้อด้วยเหตุผลที่ไม่เกี่ยวข้องกับโรงพยาบาลหรือเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า
2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ
3. หลังการสั่งซื้อ ลูกค้าจะได้รับใบเสร็จในรูปแบบออนไลน์เท่านั้น หากต้องการใบเสร็จตัวจริงในวันที่เข้ามารับบริการ สามารถแจ้งขอได้ที่เจ้าหน้าที่การเงิน

1.Service is reserved exclusively for customers who present the E-Coupon.

2.This E-Coupon cannot be exchanged for cash.

3.E-Coupon is only applied to specific items as mentioned and changes cannot be made.

4.These packages are for outpatients who are physically dependent. 

5.This E-Coupon is only applicable for scaling and polishing treatment and does not include gum disease treatment. The dentist will assess and determine the appropriate treatment for gum disease separately.

6.These packages exclude treatment fees or medications prescribed by the doctors, if any. 

7.For further information or to schedule an appointment, please contact Dental Clinic, 2nd floor, Dental Building, Bangkok Hospital, every day from 09:00 AM to 08:00 PM or Contact Center at 02-755-1335 or 1719.

8.Services are only available at Dental Clinic, 2nd floor, Dental Building, Bangkok Hospital.

9.This price is for Thai citizens and expatriates only.

 

Note:
1. In cases that customers request to cancel an order for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund or cancel the product at its discretion.
2. The receipt amount displayed is the actual payment received, excluding any promotional discount.
3. After completing the purchase, customers will receive an online receipt only. If a physical receipt is required on the day of the visit, it can be requested from the cashier.

2.   โทรนัดหมายล่วงหน้าอย่างน้อย 3 วัน ก่อนเข้ารับบริการ โทร Contact Center โทร.1719 หรือ Add Line @bangkokhospital
3.   ก่อนวันเข้ารับบริการ กรุณาเข้าไปที่ คำสั่งซื้อของฉัน -> ที่ต้องได้รับ และคลิกปุ่ม ตรวจสอบและยอมรับ
4.   แสดงคูปองอิเล็กทรอนิกส์ ให้กับเจ้าหน้าที่ในวันที่รับบริการ

1. After successful payment, you will receive an SMS with an e-coupon.
2. Please schedule an appointment at least 3 days in advance before receiving the service by calling the Contact Center at 1719 or add line @bangkokhospital
3. Before the service date, please go to "My Orders" -> "To Receive" and click the "Check and Accept" button.
4. Present the e-coupon to the staff on the day of the service.
