กรณีมีวัคซีนพร้อมให้บริการก่อนเดือนมิถุนายน โรงพยาบาลจะทำการติดต่อหรือแจ้งให้ทราบ

 

เงื่อนไขการใช้บริการ

1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำ E-Coupon มาแสดงเท่านั้น

2. สำหรับใช้บริการผู้ป่วยนอกเท่านั้น / อัตราดังกล่าวรวมค่าแพทย์ผู้ป่วยนอกแล้ว

3. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้

4. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

5. กรุณาโทรนัดหมายล่วงหน้าก่อนรับบริการอย่างน้อย 1 วัน ที่ Contact Center โทร 02-310-3000 หรือ 1719

6. สามารถรับบริการที่ คลินิกกุมารเวช ชั้น 4 อาคาร B โรงพยาบาลกรุงเทพ โทร. 02-310-3006

เปิดบริการทุกวัน เวลา 08:00 - 16:00 น. 

7. แพ็กเกจนี้ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

หมายเหตุ

1. ในกรณีที่ลูกค้าประสงค์จะยกเลิกคำสั่งซื้อด้วยเหตุผลที่ไม่เกี่ยวข้องกับโรงพยาบาลหรือเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า

2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ

3. หลังการสั่งซื้อ ลูกค้าจะได้รับใบเสร็จในรูปแบบออนไลน์เท่านั้น หากต้องการใบเสร็จตัวจริงในวันที่เข้ามารับบริการ สามารถแจ้งขอได้ที่เจ้าหน้าที่การเงิน

In case vaccines become available prior to June, the hospital will contact and notify you accordingly.

 

1. Service is reserved exclusively for customers who present the E-Coupon.

2. Without the coupon, the regular package price will apply.

3. This E-Coupon cannot be exchanged for cash.

4. E-Coupon is only applied to specific items as mentioned and changes cannot be made.

5. Valid for outpatient services only. The price includes physician fees.

6. This package does not include additional treatment or medication costs as prescribed by a physician.

7. Please make an appointment at least 1 day in advance via Contact Center at +66 2 310 3000 or 1719.

Note:

1. In case of cancellation  request for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund.

2. The receipt amount displayed is the actual payment received, excluding any promotional discount.

3. An e-receipt will be provided. A physical receipt can be requested at the cashier counter on the day of service.

3. ก่อนวันเข้ารับบริการ กรุณาเข้าไปที่ คำสั่งซื้อของฉัน -> ที่ต้องได้รับ และคลิกปุ่ม ตรวจสอบและยอมรับ
4. แสดงคูปองอิเล็กทรอนิกส์ ให้กับเจ้าหน้าที่ในวันที่รับบริการ

1. After successful payment, you will receive an SMS with an e-coupon.
2. Please schedule an appointment at least 3 days in advance before receiving the service by calling the Contact Center at 1719 or add line @bangkokhospital
3. Before the service date, please go to "My Orders" -> "To Receive" and click the "Check and Accept" button.
4. Present the e-coupon to the staff on the day of the service.
