1.ราคาดังกล่าวรวมค่าห้องผ่าตัด ค่าห้องพักฟื้น ค่าเวชภัณฑ์ ค่ายาที่ใช้ในการผ่าตัด ค่าทีมแพทย์ทำหัตถการ เฉพาะที่เกี่ยวข้องกับการผ่าตัดตามที่ระบุเท่านั้น
2.ราคาดังกล่าวรวมค่าอุปกรณ์พิเศษเลนส์เสริม (ชนิดที่กำหนด) หากหลังจากตรวจสายตาก่อนผ่าตัดและไม่สามารถใช้เลนส์ชนิดที่กำหนดได้ จะมีค่าใช้จ่ายเพิ่มเติม ทั้งนี้เจ้าหน้าที่จะแจ้งข้อมูลก่อนการตัดสินใจ
3.ราคาดังกล่าวรวมค่ายากลับบ้าน (Home Medication) แต่ไม่รวมค่ายาโรคประจำตัวของผู้ป่วย
4.ราคาดังกล่าวรวมการตรวจติดตามอาการหลังการผ่าตัด 1 ครั้ง
Terms and conditions
1.These prices include operating room, recovery room, ophthalmologist fee medical equipment and medical supply used for the surgery as specified by the hospital.
2.These prices include lenses as specified by the hospital. In the event that, following a pre-surgical eye examination, the prescribed lenses cannot be used, additional charges may apply. The staff will provide information before any decision is made.
3.These prices include home-medication after surgery, but medications for patient’s underlying diseases are excluded.
4.These prices cover 1 visit of postoperative follow-up

6.ราคาดังกล่าวรวมอุปกรณ์พิเศษเช่น เลนส์ ที่ใส่ในร่างกายของผู้ป่วย (เฉพาะหัตถการที่กำหนด)
7.ราคาดังกล่าวรวมค่ายากลับบ้าน (Home Medication) แต่ไม่รวมค่ายาโรคประจำตัวของผู้ป่วย
8.ราคาดังกล่าวรวมการตรวจติดตามอาการหลังผ่าตัด 6 เดือน (จำนวน 5 ครั้ง)

วันหมดอายุ E-Coupon
1. E-Coupon จะมีอายุการใช้ถึงวันที่ 30 พฤศจิกายน พ.ศ. 2567
2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้
3. แพ็กเกจราคานี้ สามารถสั่งซื้อได้ตั้งแต่ วันที่ 27 กรกฎาคม - 18 สิงหาคม พ.ศ. 2567
หมายเหตุ
1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Shopee Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ
2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

5.This price includes the cost of the operating room, recovery room, medical supplies, surgical medications, and surgical team fees specifically related to the specified surgery.
6.This price includes special equipment such as lenses that are implanted in the patient's body (for specified procedures).
7.This price includes the cost of home medication but does not include the cost of regular medications for the patient's existing conditions.
8.This price includes 6-month post-operative follow-up visits (a total of 5 visits).

Expiration Date of E-Coupon:
1. The E-Coupon is valid until November 30, 2024.
2. If the E-Coupon expires, the hospital reserves the right to deny service access, and the E-Coupon cannot be extended or refunded.
3.This package price can be ordered from July 27, 2024, to August 18, 2024.
Note:
1. In case customers request cancellation of the order after Health Plaza Guarantee period without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.
2. The receipt amount displayed is the actual payment received, excluding any promotional discounts.
