6. ราคานี้รวมค่าแพทย์แจ้งผล แต่ไม่รวมค่าแพทย์เฉพาะทางเต้านมในกรณีที่ต้องปรึกษาแพทย์เพิ่มเติม

7. ในกรณีผู้รับบริการที่มีอาการผิดปกติต่าง ๆ มาก่อนที่จะทำการตรวจ (อาจมีการทำหัตถการอื่น ๆ เพิ่มเติม) หรือผู้ป่วยที่มีนัดหมายเพื่อติดตามอาการจากความผิดปกติ จะมีค่าบริการพบแพทย์ตรวจเฉพาะทางเพิ่มเติมจากราคาแพ็คเกจนี้

1. Service is reserved exclusively for customers who present the E-Coupon, and please show the E-Coupon to the staff before using the service.

2. This E-Coupon cannot be exchanged for cash.

3. E-Coupon is only applied to specific items as mentioned and changes cannot be made.

4. These packages are for outpatients who are physically dependent.

5. This price applies to individuals receiving Screening for Breast Cancer who do not have any abnormal symptoms, This package is suitable for women aged 40-75.

6. This package include doctor fee. This package excludes additional counseling fee provided by breast specialists. This package excludes treatments and medications. Cost incurred besides this package will be additionally charged.

7. In the case of patients who have various pre-existing abnormal symptoms (which may involve additional procedures) or patients with appointments for symptom follow-up, there may be additional specialist doctor consultation fees beyond the price of this package.

8.Please make an appointment 3 days in advance. For further information or to make an appointment, please contact Breast Center at 02 310 3016 (operating hours: 07:00-15:00) or the Contact Center at 1719.

 

Note:

1. In case customers request cancellation of the order after the Health Plaza Guarantee period without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.

2. The receipt amount displayed is the actual payment received, excluding any promotional discounts.
